NeFora Terms of Use
These Terms of Use govern access to and use of NeFora, a digital kidney education, intake, and care-navigation tool made available by the Canadian Kidney Care Collaborative (CKCC) and, in some cases, by participating healthcare organizations using NeFora in their clinical workflows. By accessing or using NeFora, you agree to these Terms of Use.

Education and workflow support only
NeFora helps with education, intake, visit preparation, care navigation, and supported health-reading tracking. It does not replace direct clinical care.
Not for emergencies
Do not use NeFora for urgent or emergency medical issues. Contact your care team or emergency services directly when time matters.
Device sync is not live surveillance
Connecting a supported device does not mean your clinic is watching your readings in real time unless a separate monitored program has been clearly activated.
Authorized use matters
You must provide accurate information and only use NeFora for yourself or for another person when you are authorized to do so.

NeFora is a digital tool designed to support kidney education, intake, visit preparation, care navigation, and patient-entered or patient-authorized health tracking. Depending on the workflow enabled by a participating healthcare organization, NeFora may help users:
- review educational materials;
- answer intake or questionnaire prompts;
- organize symptoms, concerns, goals, and priorities;
- manually enter blood pressure, weight, blood sugar, or similar health readings;
- synchronize readings from supported health-monitoring devices or authorized third-party health accounts;
- prepare for appointments; and
- share summaries or other outputs with their care team through approved workflows.
NeFora is not an emergency service, crisis service, or replacement for direct clinical care.

NeFora provides educational, intake, and workflow support only. It does not diagnose medical conditions, provide individualized treatment recommendations, replace professional medical judgment, or establish a physician-patient relationship with CKCC, Boreal Health, or their technical providers.
Always contact your treating healthcare team directly for medical advice. For urgent symptoms, emergencies, or time-sensitive concerns, call 911 or your local emergency services, or seek immediate medical attention.

NeFora is not continuously monitored. Questionnaires, symptom check-ins, concerns, messages, synced device readings, summaries, or other information submitted through NeFora may not be reviewed immediately. Do not use NeFora to communicate urgent or emergency information.
Connecting a blood pressure monitor, scale, glucose device, or other supported service does not mean that your doctor, clinic, or care team is watching your readings in real time unless your participating healthcare organization has separately told you that a specific monitored program has been activated.

NeFora may be made available to:
- patients;
- caregivers;
- parents or guardians;
- substitute decision-makers; and
- authorized clinic or health-system users,
where permitted by applicable law and by the participating healthcare organization. You agree to provide accurate and complete information when using NeFora. If you are using NeFora on behalf of another person, you represent that you are authorized to do so.

You agree not to:
- misuse NeFora or use it for unlawful purposes;
- attempt to gain unauthorized access to any account, system, or data;
- interfere with the operation, integrity, or security of the service;
- impersonate another person or misrepresent your authority;
- upload or submit harmful, unlawful, fraudulent, or malicious content; or
- use NeFora in a way that could harm other users, CKCC, Boreal Health, or participating healthcare organizations.
7. Intellectual Property

NeFora, including its software, user interface, workflow design, text, graphics, features, and related content, is owned by or licensed to CKCC, Boreal Health, and/or their licensors, and is protected by applicable intellectual property laws.
NeFora is licensed, not sold, for your personal or authorized organizational use in accordance with these Terms of Use. Except as expressly permitted by law, you may not copy, modify, distribute, reverse engineer, republish, sell, sublicense, or create derivative works from any part of NeFora without prior written permission from the applicable rights holder.

Your use of NeFora is also governed by the NeFora Privacy Notice and Additional Disclosures, which explains what information may be collected, how it may be used or disclosed, and what choices may be available to you.
If NeFora is launched through Epic, MyChart, or another approved integration, you should review the privacy notice before authorizing the connection.
9. Availability and Changes

CKCC and participating healthcare organizations may update, suspend, restrict, or modify NeFora or its content from time to time, including for maintenance, quality improvement, security, clinical review, operational changes, or legal compliance.
Supported device integrations, third-party sync connections, and data-import features may also be changed, interrupted, delayed, or removed depending on vendor APIs, permissions, technical constraints, legal review, or operational decisions.
CKCC may also update these Terms of Use from time to time. If material changes are made, revised terms will be posted and additional notice may be provided where appropriate.
10. No Warranty

To the fullest extent permitted by applicable law, NeFora is provided on an “as is” and “as available” basis. CKCC, Boreal Health, and their collaborators do not guarantee that NeFora will always be available, uninterrupted, error-free, compatible with every device or clinical system, or free from delays, omissions, or defects.
This includes connected device and third-party sync features. Imported readings may be delayed, incomplete, duplicated, missing, or unavailable, and manual entry may still be needed.
11. Limitation of Liability

To the fullest extent permitted by applicable law, CKCC, Boreal Health, participating healthcare organizations, and their collaborators will not be liable for any indirect, incidental, special, consequential, or similar damages arising from:
- misuse of NeFora;
- internet, device, platform, or third-party system failures;
- device-sync interruptions, data-import delays, missing readings, duplicate readings, or vendor API changes;
- delays or interruptions in availability;
- unauthorized use caused by your failure to safeguard credentials or devices; or
- reliance on NeFora as a substitute for direct clinical care.
Nothing in these Terms of Use excludes or limits any liability that cannot be excluded or limited under applicable law.
12. Governing Law

These Terms of Use are governed by the laws of the Province of Ontario and the federal laws of Canada applicable there, unless mandatory law in another province or territory requires otherwise for a particular user or deployment.
13. Québec Availability

NeFora is not currently offered to Québec residents during the English-only launch phase. If and when NeFora is launched in Québec, required user-facing terms, privacy materials, and core app interfaces will be made available in French in accordance with applicable law.
